 Description   

 01. Fix rmfid silently accepting fid without fsname
        or lustre root mount point.      
   

$ lfs rmfid 0x200000402:0x1:0x0
$

    
 02. Fix rmfid unable to handle fsname. Would only
        work with lustre root mount point.

$ lfs rmfid lustre [0x200000402:0x2:0x0]
lfs rmfid: cannot remove FIDs: Inappropriate ioctl for device

    

 03. Fix rmfid memory leak
