[LU-2512] b1_8 and b2_1 test scripts fail against 2_3 and master server

Description

The b1_8 and b2_1 scripts do not provide the index parameter for mkfs and so fail against a b2_3 or later server (I have not explore 2_2)

This is the problem reported in LU-1526 already, and is one of the reasons why I'd like to be testing interop against b2_1 and b1_8 instead of 2.1.3 and 1.8.8, respectively.
