This is related to but not caused by LU-2749. The cause of this bug seems to be osd-zfs/osd_io.c::osd_oi_create() returning EEXIST to osd-zfs/osd_io.c::osd_find_or_add_seq on the OST, which causes the MDT to assume that the OST is further more insane.
