Third level headings 14.8.3 and above appear in the main TOC but not in the list of contents that appears at the head of Chapter 14. Headings 14.8.1 and 14.8.2 appear both places. This seems like an infrastructure issue rather than a content issue so assigning it to Richard Henwood. |
