[LUDOC-192] Lustre manual doesn't open in some browsers

On IE 8 for Dan Cobb. On Firefox for Linda Bebernes. Other browsers not tested.
(Linda) When an attempt is made to open the xhtml version of the Lustre manual in Firefox, an error message is displayed:

(Linda) When an attempt is made to open the PDF in firefox, the PDF frame appears but no TOC or content.

(Dan) When an attempt is made to open the xhtml file in IE 8, an "unknown file type" error message is displayed. [the xhtml file opens in Linda's IE 8 browser)

(Dan) When an attempt is made to open the PDF in IE8, it doesn't load. (It loads fine in Linda's IE 8 browser).
