Remove lsm_lock

    Description

      The lsm lock was formerly used to protect the lov_stripe_md struct. That use has been replaced by the coh_attr_guard lock.

      So, we should remove it. (This reduces the cost of lov_attr_get enough to be measurable in the tiny writes patch, https://review.whamcloud.com/#/c/27903/)
